Immunizations and Health Requirements for School Attendance

Physical Exams – NYS Physical Form

If your child has a well-child physical, please be sure to request a copy for the school nurse. Starting the 2018-19 school year, NYS Education Law requires a Physical Exam (well-child exam) for school attendance for students in grades K, 1, 3, 5, 7, 9 and 11 and all new entrants. However, all physicals are welcome throughout the year.

  • Documents must be signed by the health care provider and submitted upon registration or before the school year begins.
  • Forms from a health care provider's office are accepted if they contain the required information and are signed by the provider.
  • MyChart and MyCare visit summaries are not accepted as Physical Health Exam documents.

 

Immunizations

NYS law dictates the required vaccines required for school entrance. Please refer to the information provided below regarding the specific requirements per grade level. 

Dental Exams – Dental Health Certificate

Starting the 2018-19 school year, NYS Education Law requests a Dental Certificate for students in grades K, 1, 3, 5, 7, 9 and 11 and all new entrants.

  • Public Health Law Section 2164 and New York Codes, Rules and Regulations (NYCRR) Title 10, Subpart 66-1 require every student entering or attending public, private or parochial school in NYS to be immune to diphtheria, tetanus, pertussis (DTP), measles, mumps, rubella (MMR), poliomyelitis (IPV), hepatitis B, and varicella (chickenpox) in accordance with Advisory Committee on Immunization Practices (ACIP) recommendations.

    • Students in grade 6-12 are also required to be vaccinated against pertussis with one dose of Tdap.
    • Students in grades 7-10 and 12 are also required to be vaccinated against meningococcal disease.
    • Students in preschool or prekindergarten are also required to be immune to Haemphilus influenzae type b (Hib) and pneumoncoccal disease (PCV).
  • Medical exemptions must be renewed annually by a NYS licensed physician and submitted to school nurse yearly. Our district physician has final approval of all medical exemption requests. The Medical Exemption Form must list the immunization exempted, the medical contraindication and the length of time the immunization is contraindicated. Serologic proof of immunity can be accepted ONLY for the following diseases: measles, mumps, rubella, varicella, and hepatitis B. A health care provider’s signed statement attesting to the diagnosis of varicella is acceptable proof of immunity for varicella.

  • On 06/13/19, New York Gov. Andrew Cuomo signed a bill ending vaccination exemptions based on religious beliefs. NYS will now only allow medical exemptions from immunizations as a valid reason for not immunizing a child or adolescent for attendance to school. Please understand that the district must follow all NYS laws and regulations. At this point, we are awaiting guidance from the State Education Department about when and how to implement this new law.
